If you or your loved one are in need of our services, please follow the steps below to apply.
Contact our Intake Coordinator, Stacy Cornett 936.271.2183 and leave a detailed message with the following information:
A client liaison will return your call within 10 business days. An over-the-phone assessment will be given to determine eligibility. For those who qualify for meal delivery, an in-home assessment will be scheduled (alternative options available due to COVID-19).
